Care navigation service

Our care navigation service is a recognised centre of excellence, that provides safe and appropriate referral support using the most clinically appropriate providers.


The services within the Care Navigation Service (CNS) include:

  • Patient Transport Advice Centre (PTAC). This is a single point of contact to assess a patient's mobility and health needs to identify the most appropriate transport options when travelling to their healthcare appointment. Where patients are assessed as eligible for NHS-funded Non-Emergency Patient Transport Services (NEPTS), PTAC will make the transport bookings with the transport provider. PTAC will signpost to alternative transport options for those that do not meet the eligibility criteria. This service is currently delivered to patients registered at GP Practices within Somerset, Dorset, Gloucestershire, BANES, Wiltshire and Swindon.
  • Directory of Services (DoS) Hub, where several ICB customers access system support and subject matter expertise for DoS to support Urgent and Emergency Care (UEC) pathways.
  • The Elective Referral Management team provides support and engagement to GP practices within Somerset regarding NHS e-Referral systems, Advice & Guidance and other assessment pathways, including Dermatology. This team also provide systems for capturing Somerset Optometry claims and referral metrics.
