Supporting the world’s first diabetes prevention programme with a contract management function

Ensuring participating providers efficiently provide tailored, personalised help to reduce the risk of Type 2 diabetes.

The challenge

Diabetes is a leading cause of early death with over 22,000 deaths each year. Having diabetes doubles an individual’s risk of cardiovascular disease and can lead to short or long-term complications including problems with the eyes, heart, kidneys, nerves and feet. It also shortens average life expectancy.

There are now over 3 million people in England with a diagnosis of diabetes and approximately another million living with Type 2 diabetes who are yet to be diagnosed.

The Healthier You: NHS Diabetes Prevention Programme (DPP) is a joint initiative between NHS England, Public Health England and Diabetes UK and is the first nationally offered programme in the world and is offered as part of the NHS Long Term Plan’s renewed focus on prevention.

SCW was appointed to deliver contract management services for the programme to manage the DPP provider contracts to monitor provider contracts and performance against KPIs (Key Performance Indicators), keep their planned levels of activity and consequent costs under review and provide update reports on progress and risks to the central team.

 Our approach

  • SCW provided expert contract management services to manage the five DPP providers contracts through the following:
  • Management of the dataflows through our DSCRO.
  • Produced and reviewed KPI reports and managed the performance of contracts
  • Produced monthly activity reporting to support contract management
  • Provided monthly site-level reports to facilitate discussion and performance management at the regional and STP level
  • Provided the finance and payment functions to providers
  • Maintained excellent relationships with the providers and managed issues effectively
  • Oversaw and coordinated the contract variation process for all contracts
  • Provided administrative support to a range of regular contract review meetings and other meetings as required.

 The impact

  • The contract management function of the DPP became well established over the period of the contract and has resulted in the contract being extended and expanded from 2020.
  • The programme purchased high-intensity behavioural interventions at low prices and made these available to people at high risk of diabetes to support behaviour change at scale.
  • We developed and revised the Minimum Data Sets (MDS) in the 2019 framework revision to include the addition of digital services and introduced the data landing platform.
  • In 2017/18 the programme delivered an estimated 130,000  referrals and around 50,000 people participating in behaviour change programmes
  • In 2018/19 this rose to 200,000 referrals and 80,000 people on programmes.
